Future Clinical Services
Uplift Path has applied for CARF (Commission on Accreditation of Rehabilitation Facilities) accreditation to provide Community Psychiatric Supportive Treatment (CPST) services via telehealth for adults aged 18+ with active Medicaid coverage. These clinical services will be available only after:
CARF accreditation approval
Ohio Mental Health and Addiction Services (OMHAS) licensure
Medicaid provider enrollment completion
Formal announcement on this Website

- Our use and disclosure of PHI will be strictly governed by HIPAA regulations, state law, and our Privacy Policy.

Indemnification
You agree to defend, indemnify, and hold harmless Uplift Path Parties from and against any and all claims, liabilities, damages, losses, costs, and expenses (including reasonable attorneys' fees) arising out of or related to:

Dispute Resolution and Arbitration
Informal Resolution
Before initiating any formal dispute resolution, you agree to contact us at info@upliftpathinc.com to attempt to resolve the dispute informally. We will attempt to resolve disputes in good faith.
Binding Arbitration
If informal resolution is unsuccessful, any dispute arising out of or relating to these Terms or the Website shall be resolved through binding arbitration in accordance with the American Arbitration Association's (AAA) Commercial Arbitration Rules, except as modified by these Terms.
The arbitration shall be conducted in Franklin County, Ohio, and judgment on the arbitration award may be entered in any court with jurisdiction.
Exceptions to Arbitration
Notwithstanding the above, either party may seek injunctive or equitable relief in court to prevent infringement of intellectual property rights or confidential information.
Class Action Waiver
YOU AGREE THAT ANY ARBITRATION OR PROCEEDING SHALL BE LIMITED TO THE DISPUTE BETWEEN YOU AND UPLIFT PATH INDIVIDUALLY. TO THE FULLEST EXTENT PERMITTED BY LAW, YOU WAIVE ANY RIGHT TO PARTICIPATE IN A CLASS ACTION LAWSUIT OR CLASS-WIDE ARBITRATION.
