Version 1.0 · Effective October 2026
Notice of Nondiscrimination and Availability of Language Assistance and Auxiliary Aids
Uplift Path, Inc. complies with applicable federal civil rights laws and does not discriminate, exclude people, or treat them differently on the basis of race, color, national origin, sex, age, or disability in its health programs and activities.
Uplift Path, Inc. provides, free of charge and in a timely manner:
Reasonable modifications to its policies, practices and procedures for people with disabilities;
Appropriate auxiliary aids and services for people with disabilities, so that communication with us is as effective as communication with anyone else — including qualified sign language interpreters, live captions, and written material in other formats such as large print, audio, accessible electronic files and plain language;
Language assistance services for people whose first language is not English — including qualified interpreters and written information translated into other languages.
You will never be denied services or asked to pay for these supports, and we do not require you to bring your own interpreter. We do not rely on a minor child to interpret for you. We may rely on a family member or friend to interpret only if you specifically request it and we determine, consistent with 45 CFR 92.201(c), that it is appropriate in the circumstances; otherwise we provide a qualified interpreter.
If you need any of the supports described above, tell any member of our workforce, or contact us:
Telephone: (513) 299-4553 Ohio Relay / TTY users: dial 711
Email: advocate@upliftpathinc.com or Online: upliftpathwellness.com/contact
We will acknowledge your request in a timely manner — we estimate up to ten (10) business days — and arrange the support you need.
If you believe Uplift Path, Inc. has failed to provide these services, or has discriminated against you, you may raise a concern at grievance@upliftpathinc.com, or by completing a grievance form on the website, or by telephone on (513) 299-4553. We will acknowledge your concern in writing and attempt to resolve in line with our Client grievance policy and procedure. If you need assistance in completing the grievance form you may request a Client Rights Specialist at the same email. Raising a concern will never affect the services you receive.
You may also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. You may do this whether or not you raise the matter with us first.
