Cookies and Tracking Technologies Notice

Version 2.0 · Effective October 2026

Introduction

This Notice explains the cookies and similar tracking technologies used on upliftpathwellness.com (the “Website”), what each category does, who receives the information, and how you can control them. It is published separately from our other privacy notices so that it can be linked directly from our cookie banner and updated whenever a tracking technology on the Website changes.

Who This Notice Is for

This Notice applies to everyone who visits the Website, whether or not you go on to ask us about services.

Uplift Path, Inc. provides behavioral health services to adults. We do not provide services to children or adolescents. The Website is not directed to children, we do not knowingly collect information from anyone under thirteen, and our service inquiry form is for adults aged eighteen or over. If you believe a child has given us information through the Website, contact us using the details at the end of this Notice and we will delete it.

What Cookies and Tracking Technologies Are

Cookies are small text files placed on your device when you visit a website. Similar technologies include web beacons, pixels, tags, software development kits, local storage and session replay scripts. Together these are referred to in this Notice as tracking technologies. They can record what you viewed, how long you stayed, what you clicked, and what device and network you used.

Categories We Use

A. Strictly Necessary

Required for the Website to function and for security. They are set in response to actions you take, such as navigating pages, completing a form or setting your privacy preferences. These cannot be switched off through our banner because the Website will not work without them.

B. Analytics and Performance

Help us understand how visitors find and use the Website so we can improve navigation, content and reliability. Where the provider supports it, internet protocol address anonymization is enabled and data retention is set to the shortest period the provider allows. These are off by default until you consent.

C. Functionality

Remember choices you make, such as language or display preferences, so you do not have to set them again. These are off by default until you consent.

D. Advertising and Social

Where used, these allow advertising and social platforms to measure campaigns and to show advertisements. They are off by default and require your affirmative consent before they load. They are never loaded on our service inquiry form, and they never receive anything you enter into it. See the next section.

Our Service Inquiry Page for Adults

Anyone may visit this Website and ask us about services, including adults enrolled in Medicaid. You do not need a referral and you do not need an appointment to make an inquiry. We want that to be straightforward, and we also want it to be private. This section explains how we separate the two things that happen there.

Visiting the page. Looking at a page that describes our services, including the inquiry page itself, is treated like any other page visit on the Website. Your cookie preferences apply in the ordinary way, and if you have not consented to a category, nothing in that category loads.

Submitting the form. The moment you enter your details and submit an inquiry, different and stricter rules apply, and they do not depend on your cookie choices:

  • Nothing you type into the form is transmitted to any analytics, advertising or social platform. Not your name, contact details, Medicaid status, or anything you tell us about why you are contacting us.

  • Advertising and social tracking technologies do not load on the form at all, whatever consent you have given elsewhere on the Website.

  • Web address parameters and referrer information from the form are not passed to those platforms, so the fact that you submitted an inquiry is not shared as an advertising event.

  • The form is hosted on a secure platform operating under an executed Business Associate Agreement, separately from general Website analytics.

  • From the point of submission, the information you give us is health information held by a healthcare provider. It is governed by our Client Notice of Privacy and by federal and state health privacy law, not by this Notice.

What We Do Not Do

These commitments apply across the whole Website and are not affected by your cookie choices:

  • We do not sell your personal information or your health information.

  • We do not share health information, or information suggesting that you are seeking behavioral health services, with advertising or social platforms.

  • We do not use health information to build advertising audiences, and we do not target advertising to you on the basis of a health condition, a diagnosis, or the fact that you contacted us.

  • We do not use geofencing around any healthcare facility to identify or advertise to people near it.

  • We do not require you to accept non-essential cookies in order to reach information about our services or to submit an inquiry. Declining changes nothing about the care available to you.

Managing Your Preferences

A cookie banner appears on your first visit and lets you accept or decline each non-essential category. You can reopen the banner at any time from the link in the Website footer to change or withdraw your choices, and withdrawing consent is as easy as giving it. You may also block or delete cookies through your browser settings, although disabling some categories may affect how the Website works.

Opt-Out Preference Signals

We honor browser-based opt-out preference signals, including Global Privacy Control. Where we recognize such a signal we treat it as a valid instruction to switch off non-essential tracking for that browser, and we do so wherever you are located rather than only where a law requires it. Our Website does not respond to legacy Do Not Track headers, which are not supported by a common standard.

Your Choices and Rights, Wherever You Are

Privacy laws differ from state to state, and some states give residents specific rights over health-related information collected online. Rather than apply different standards to different visitors, we apply the commitments in this Notice to everyone who uses the Website. If you want to know what we hold from your use of the Website, ask us to delete it, or withdraw a consent you previously gave, contact us using the details below and we will act on your request.

Third-Party Providers

Analytics, hosting, form and advertising providers receive only the categories of information described above, and only for the purposes we authorize. Each provider processes information under its own terms and privacy policy in addition to our contract with it. Any provider that will handle information protected by health privacy law must execute a Business Associate Agreement before it receives any such information. We do not place a provider on the inquiry form unless that agreement is in place.

How This Notice Relates to Our Client Notice of Privacy

This Notice covers your use of the Website up to the point you submit an inquiry. Our Client Notice of Privacy covers the health information we hold about you as a healthcare provider, from that point onward and throughout any services you receive. Where the two could overlap, the Client Notice of Privacy and health privacy law govern.

Changes to Tracking Technologies on This Website

Before any new tracking technology is added to the Website, it is reviewed against this Notice and our vendor agreements, and the outcome is recorded in our internal Monitoring Audit Register. That review is approved by our HIPAA Security Officer, with our HIPAA Privacy Officer consulted where the change would affect what is disclosed about a visitor. Our Chief Risk Officer maintains oversight of this Notice as a policy of the organization.

If we change this Notice, we will publish the updated version here with a new effective date, and we will describe what changed. A change applies from the date it is published and we do not apply it backward.

Record Retention and Review

This Notice is reviewed at least annually, and whenever the Website, our vendors, our tracking technologies, our service area or applicable law changes. Each published version is retained for at least six years.
